Live broadcast surgery is an innovative method of remotely introducing new operative techniques to surgeons globally. However, due to the potential impact on patient outcomes, its use remains somewhat controversial.The objective of this study was to determine the effect of live surgery on operative time and perioperative complication rate in patients undergoing arthroscopic anatomic glenoid reconstruction (AAGR) for shoulder instability.This was a retrospective review of 94 patients who underwent AAGR between 2013 and 2023. A 1:1 ratio was used to match patients who underwent live virtual AAGR (live broadcast group) to patients who underwent AAGR without live surgery (no broadcast group), based on sex, body mass index, and age.We hypothesized that live surgery would maintain a similar surgical time and intraoperative and postoperative complication profile.

Four studies assessed the educational value of LSB via survey administration, for which the evidence is limited and of low quality. Thirteen guidelines and position statements on live surgery were identified among major surgical societies, including the European Association of Urology (EAU).Only one study on gastrointestinal surgery reported that LSB outcomes were worse. Ongoing concerns have led to specific guidelines by several scientific societies, including the EAU, with the ultimate aim of minimizing surgical risks and maximizing patient safety. Live surgery events are often part of surgical conferences.This process fostered the adoption of live surgical broadcast (LSB), more commonly known as live surgery, as a relevant part of many types of surgical meeting worldwide [3]. However, this same process generated a lot of controversy related to ethical issues around LSB, mainly involving patient consent and safety given the risk of potential complications [4]. Many concerns were brought to light following the unfortunate events that occurred in 2007 in Japan following the death of a patient during a live cardiothoracic surgical broadcast of repair of an aortic aneurysm [5].The review was conducted according to guidelines defined in the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement [6]. The literature research was performed using MEDLINE (PubMed), Embase, and the Cochrane Database of Systematic Reviews to identify relevant studies up to December 2020. By this, a surgeon’s learning curve is vaulted far ahead of what could be achieved any other way except by visiting the surgeon. It takes the error out of trial and error. Lately though, some surgeons have been saying, “why not just have videos?” This is a good question. To arrange a live surgical demonstration, you need a reliable patient who is willing to have surgery at low or no cost.The day before surgery, the visiting surgeon will assess the patient, adding information to the host surgeon’s consultation if needed. If the initial patient declines surgery at the last moment, it is advisable to have a backup patient ready, so that the live surgical demonstration can proceed as planned.It is a direct way of comparing our techniques with those of our well-known colleague leaders, particularly in the less commonly performed operations and when there are multiple choices where no one approach seems obviously superior. Live surgery gives an immediacy not found in edited surgery and gives the audience access to ask questions not afforded by any other medium.You need a well-known surgeon who is willing to demonstrate a difficult operation, a facility, an audiovisual department and a moderator. However, are we not making this look far more sensational in its ‘live’ avatar?Using web conferencing as a tool, beaming live surgeries has become one of the most attractive and efficient way to demonstrate, explain and teach surgical or medical techniques to an audience of peers watching it from various locations, often continents apart!This exercise provides a dynamic educational platform for the audience through which knowledge is shared by the experts performing these surgeries with first hand visuals of the operation and the two-way communication with the participants during the live procedures.As participants get an opportunity to observe the live procedure with decisions taken on real time, they are also exposed to unexpected challenges and other unforeseen intricacies of a surgery. However, it is from these unforeseen and unexpected challenges that my objections to these live surgical workshops take birth.

We want to report our experience in organizing the last 7 shoulder LSE over the past 12 years focusing on the results and complications during and after the surgeries performed during these courses. 190 patients benefited from live shoulder surgery between 2005 and 2017.Live Surgery Events (LSE) have recently been criticized in France, arguing that unnecessary risks have been placed on the patients.The mean follow-up was 7,5 years for the instability group, 6,7 years for the rotator cuff group, 7,5 years for the arthroplasty group and 6,8 years for the nerve group. This study is a retrospective analysis of prospectively collected data. We report the epidemiology of the surgeries perfomed and analyzed the patient outcomes: peri and postoperative complications.
